
Please let us know immediately if you receive:
- a letter confirming a change that you didn’t ask for
- a request for personal information that seems unusual or you suspect isn’t from us.
To make it easier for you to tell what’s genuine, we’ll usually send you:
- your yearly benefits statement
- confirmation of any transactions and/or changes to your plan that you've requested
- confirmation of a change of address that you've told us about
If we've lost contact with you, we’ll try to trace your new address so we can continue to send important plan information to you. In these cases, we’ll send letters asking for the information we need to confirm the recipient's identity.
We may also follow this up with a phone call if we need to clarify any of the information.
